Forrest Glen is a recently renovated extended care facility
that offers a spectrum of care options. Each is designed to deliver the appropriate level
of services for specific needs - now and in the future.
Long-term Care
- For older adults and others requiring skilled nursing care and personal assistance for
an extended period. Along with these services, we offer a daily variety of cultural,
educational, and recreational activities to help each resident reach or maintain their
highest capability level.
Post-acute Care
- For individuals discharged from an acute care setting who require round-the-clock
skilled nursing services and rehabilitative therapies. Typically patients return home or
to the next appropriate level of care in a matter of weeks. To ensure that progress will
be maintained or improved, we teach families and caregivers necessary home care skills,
evaluate the home environment, and provide referrals to community resources.
Respite Care
- For those who require daily skilled nursing care or rehabilitative therapies for a brief
period when family or caregivers are away or simply need time to rejuvenate.
